Health Conditions InformationWe want to keep our students safe during the school day. In order to do that, it is important that we have updated information on your child. The schools require new/updated forms each school year. Please try to have all forms to the school at least 2 weeks prior.**NEW** We now have fillable health history forms, so you can save them to your computer and easily update them each year. The fillable forms do NOT go to the school directly and will need to be printed or downloaded and emailed.If your child has medication during the school day, a Medication Authorization Form is required and needs to be filled out by a healthcare provider and parent/guardian. This includes ANY over-the-counter (OTC), prescription, or emergency medication (i.e. Epipen, Glucagon). Some exceptions apply to secondary students.
